The
mission of the Extra-Mural Program is to provide high
quality, cost-effective, comprehensive community health
care to prevent illness or impairment, maintain or improve
function and health or to enable individuals with terminal
illnesses to remain at home. Health care services in
the region are provided to individuals of all ages in
their home or community with the goal of achieving independence.

The Extra-Mural
program is designed to:

Provide
an alternative to hospital admissions |

Provide
an alternative to or postponement of admission to a nursing
home |

Provide
long-term continuous health care to chronic invalids
|

•
|
Provide
long-term continuous health care and rehabilitation to the
disabled of all ages |

•
|
Provide
care at home for persons terminally ill |
|
•
|
Provide
assessment and rehabilitation services to the elderly
|
|
•
|
Facilitate
the coordination and provision of support services either
directly or indirectly (i.e., through other agencies)

The administration
of the Long Term Oxygen Program is also part of the services offered.
Assessment:
EMP in partnership with Family and Community Social Services and
the Department of Health and Wellness (Mental Health Division),
conduct assessments of clients (over 19 years) as part of the
Provincial Long Term Care Program.
Coverage:
Services provided to patients are paid by the Province
of New Brunswick for all residents registered with Medicare. Health
service/care is provided to individuals of all ages who can be
safely cared for in their home or community by a team of health
professionals with the goal to improve quality of life and/or
achieve independence. To achieve this goal, patients and/or family
members are invited to participate in the care that is being provided.
Some examples of the care provided include post-operative follow
up, respiratory care, coordinated diabetic care, post stroke care,
wound care, administration of IV antibiotics, pain management,
amputation care.
The team consists
of nurses, physiotherapists, occupational therapists, respiratory
therapists, dietitians/nutritionists, social workers, and speech-language
pathologists. This team works in collaboration with physicians.
Depending on the service/care required, one or more members of
the team may visit a patient.
School
and Nursing Home Services: Members
of the team who provide Rehabilitation Services also deliver services
to children in the schools and residents in nursing homes, in
close collaboration with the personnel in these environments.
Training:
The team provides a home care learning environment for university
students from all health discipline areas.
Hours
of Operation: Staff provide care on a visit basis and
are accessible to patients admitted to the New Brunswick
Extra-Mural Program, via telephone, 24-hours a day, 7 days
per week.
